Listed below are the insurance plans that our office accepts:

If you have an insurance card, please bring it with you so that the Front Desk Receptionist(s) may make a copy for office records.

If you do not have an insurance card, please make sure you write down your group number and social security number and date of birth of the responsible party. If this is not provided to us, then your claims can not be submitted. Payment is expected as services are rendered.
